Fees and Payments
Co-Payments and Balances due from Deductibles and Co-insurance amounts are the patient’s responsibility and are due at the time of your appointment. An administrative fee will be accessed on your account if you fail to do so. We accept payments in the form of Cash, Check, Debit or Credit cards. You can pay your bill online.
First time users will need an invitation from CIM to the Patient Billing Portal. Please ask the front desk at check-in to invite you to the portal or you can call the billing department at 678-205-1313. Your email will be used to send you the invite and you will have to create an account.
Our apologies but we do not accept Medicaid as your Primary or your Secondary insurance and we are considered out-of-network and unable to see you as a patient.
